This is definately an Evergreen International 747 safety card, I have 2 different ones looking nearly the same in my collection but both are for the Classic 747-100/200 and definately not for a SP (Only 4 exits on the main deck instead of 5 on each side for the SP). Can you confirm that the card you have is indeed for a SP and if this is the case I have no clue who would be the Operator of the aircraft (not Evergreen anyway). Interesting, curious...

I can verify that it's a Evergreen card !
I have 4 different types; some are in color, others in b&w and different sizes. The reason for not printing the Evergreen name on the cards is that the 747's were often (wet-) leased to carriers (such as Garuda, Saudia or the US government).
