Hi guys

Over the past four years I have noticed many new websites/facilities popping up to support our hobbies with Flight Simulator but I feel there is a function missing!

I can’t seem to find a website where I can log down information about in flight incidents or crashes, for example, engine failures, engine fires, decompression or maybe just crashing on take off.

I think it would be a good idea to start a website that can collect this information and start a database of incidents.

I have five pilots, two Captains and three First Officers who have agreed to study a certain amount of reports in their free time and then make recommendations so in future the incident/crash can be better avoided.

If an FS Pilot is involved in an incident either on line (for example on Vatsim) or off line you can upload this report to my website where other users will be able to discuss the incident and make recommendations. At this stage the five real world pilots will read the initial report and make their recommendations where necessary. With the users permission (who made the initial report) the real world pilots will also include in their report what and who caused the incident / crash and if it could have been avoided.

The websites primary objectives will be to improve the safety of flights and to educate other FS pilots through publishing reports of incidents and crashes with in FS (both on line and off line).

To ensure we get as much information as possible below are a few ideas I have had, but if you think I am missing something message me and let me know (message details are listed at the bottom of this message)

If possible can you give me a report of an incident (or crash) you have had in the past ten years, once again detail would be good! Please post any information you have in this forum using the information below for hints…

Please include the following information (where possible)

Airline (for example, Virgin Atlantic Airways)
Aircraft model (for example, Boeing 747-41R)
Flight Number (for example, VS 201)
Departure Airport (for example, Newark EWR or KEWR)
Published Arrival Airport (for example, London Gatwick LGW or EGKK)
Alternative Arrival Airport (for example, Bournemouth BOH or EGHH)

Published Departure Time (for example, 2100 GMT)
Actual Departure Time (for example, 2105 GMT)
Reason for Delay (for example, de-icing)

Published Flight Duration (for example, 6h40m)
Estimated Flight Duration (for example, 6h10m)
Actual Flight Duration (for example, 5h52m)

Aircrafts operational status (for example, report on slightly higher vibrations from engine one during take off and climb)

Estimated CRZ altitude (for example, FL350)
Estimated CRZ speed (for example, m.810)

Note the time when the first incident happened and what happened (for example, 23:55Z engine one temp higher then normal, vibration higher also)

Action taken at time? (for example, none – or reduced thrust to see if this correct problem)

Then carry on to say what happened, to your flight, what time you declared an emergency?, what time you started a divert?

Include as much information as you can about what you did and what information was displayed by the aircraft.

I will be releasing information about the service at www.avsim.com on Sunday 20th November 2006 and the basic website will go live around a week after that. The interactive website will go live around a week before Christmas.

That's usually one of the great aspects of a virtual airline; for example at MetroAir, pilots file a PiRep (Pilot Report) which includes a notes field in which the pilot can mention any sort of technical failure or other problems. We also have forums where details can be posted.

I think the site would be aimed at FS pilots who 1. May not fly for a VA 2. Would like the opinion of a real world pilot 3. A site dedicated to incidents with searchable databases etc…

Thank you for the link though, as an ex member of BAV I know VA’s quite well but I am one of the growing Flight Simmers who are going it alone.
