Dave there is a book called "BUSINESS AND LEGAL FORMS FOR PHOTOGRAPHERS written by TAD CRAWFORD.
It's a good piece of info and all the forms it discusses are included in the back of the book on a CD. It was only $25 list price, though I think I got it cheaper through Amazon.com.

Thank you both for the assistance. Due to time contraints (they needed it ASAP!), I ended up writing up a short license agreement, that the advertising agency was agreeable to. When I have some more time, I would like to add to it, and create a template that can be shared with others on this site.
If anyone has experience with photo use license agreements, and would be willing to give me some feedback on the document, please contact me via profile. I would greatly appreciate it and will of course share it with anyone once it is completed.
