Release Statments For Others To Use Your Photos

When typing up the release statement saying you'll let a certain someone use your photos what should be included in the letter? Something like: Mr. X has the right to use my photo of ____________ for promotional purposes for ______ Airlines? I would like to know because this will be the first one I've done. I'm letting an airline use the photo for promotional purposes. Thanks for any tips.

It should say how many times they can publish it, what media, if or not your name should be printed nearby...
