VS first, then anyone else with flat beds (although I've read a lot of good things about AF lately), stay away from carriers that do not have a flat bed product.
As much as I hate to say it, the Europeans tend to do better with "onboard service" than American carriers; that said, if you get any of the Americans carriers on a good day, they can do just as well, if not better.
I guess destination will drive a lot of your decision making.

I would have to agree with Rob, except that I would try AF over BA at the moment, just because of the fact that they just redid their First class and it looks nice, and I've heard good things about it.
Also if you don't mind going through the middle east I would take EK with out a second thought.
