For a relatively small airport, we're spoiled for choices in airlines at BHM. We have:
Northwest (mostly BAE 146s/Avro RJs, which seem exotic in a land of MD-80s and 737s)
US Airways & America West
I can't think of another one we need.

Why might you ask: Because I don't particularly care for WNs cattlecar environment and I'd like to see an LCC get into ANC to help drop fares. US comes here, but not with enough frequency and they (currently) suck for on time performance and just about everything else (except in cabin service). So they aren't a factor, one trip r/t ANC on US and no one wants to fly them again.
F9 already serves ANC seasonally. If there was a market, I'm sure F9 would go year-round. There is no market for that.
QQ does not, and likely will not. Much more fiscally responsible of AS to "codeshare" and "partner" with locally owned and operated carriers such as PenAir and Era. No loss to AS if either of those two run into trouble (like Era, which is currently under bankruptcy protection).

Realistically speaking (without the need for a runway extension) it would be great to have the following airlines at Plymouth;
Isles of Scilly Skybus with their DHC-6's to St Mary's.
Aurigny Airways with ATR42's to Guernsey.
KLM could use the F50 to fly to AMS.
VLM with the F50 to LCY. (Have actually heard some rumours about this one being true)
Aer Arran using the ATR42 for Dublin flights.
Eastern Airways - destinations like Birmingham and some in Scotland spring to mind.

At PHX, it would be nice to see more European carriers (LH come back, AF, KL) and even Asian carriers. But PHX competes with LAX/LAS and SAN. Eventhough PHX is bigger than the latter two, population wise.

Basically, from Barcelona, we need MORE TRANSCONTINENTAL FLIGHTS!!!
- Need flights to/from Asian destinations as BJS, PVG, TYO, CAN, BKK and SEL;
- Need another carrier to serve MOW (supposed to be announced in few days by Clickair) and other ex-Soviet cities (TBS, GYD, ALA, TAS, LED, RIX, ASB,...)
- Need many carriers to serve Africa (not only Morocco, Tunicia, Algiers and Gambia): Kenya, Ethiopia, Senegal, Mali, South Africa and other countries have a great demand to direct flights.
- Need another carrier to serve South America (most reliable than Air Madrid)
And Iberia CEO's (Fernando Conte) continues with it's monologue that BCN have not enough demand to put any transcontinental flight (here we have an idiot person). Let's remember when IB had a 763 flying daily from BCN to JFK. Loads factors of this flight were 97%!!!!!!

Taca/Copa-We have a large Central American population here.
Frontier-From what I heard, they did quite well before the storm.
Midwest-They also did well
Alaska-We need more west coast non-stops, SEA or PDX would be a nice compliment to this city
US Airways "west metal"-they need to bring back their non-stops to PHX and LAS, WN's frequency and schedule is horrible

Although I agree, I wonder if CO could even sustain such a route given that B6 already serves JFK? What would really benefit me would be a NW nonstop to DTW. However, again, I'm not sure if the market is there.
My guess as to why FL doesn't yet serve SMF would be because they don't have a very large presence on the West Coast, and they already serve SFO. Sure would be cheap having WN, B6, F9, and then FL!
Also, I note your in Folsom. Excellent choice, happened to be mine as well.
