If it's the latter, I've flown to SFO (my home airport is SMF), where I connected to a flight to OGG.
If we're only talking final destinations, then it would be BWI when I was living near RDU.

When I was living in Greece, I often flew OA between ATH and JSY (Syros) which is a total distance of 65 nm. The equipment was Beech 1900D I believe.

Both of my homes are exactly the same distance from the closest airport I have ever flown to:
SAN-LAX = 95 nm
BFL-LAX = 95 nm
I did BFL-LAX three times - UA 727-100 (BFL-LAX) and UA 737-200 (LAX-BFL) in December, 1974/January, 1975, and once on an American Eagle J31 (BFL-LAX) in July, 1992.
I have done SAN-LAX on a UA 737-300 in the days of Shuttle by United, a CRJ-200, and an EM2, but in return only on EM2's.

From PHL I flew to PVD nonstop on a WN 73G and another time on a US express (RP) E170. As a connecting hub it would either be CO ex routing ABQ-EWR-PHL (62nm) or UA routing PHL-IAD-MXP-IAD-PHL (I believe on a CR2) which is a bit farther (I also forget the express carrier as this was in 2001).. I also flew to BOS once on US (752 out and A319 back).

SMF-SFO was 86 miles, EMB-120, Skywest dba United Express.
RDU-BWI was 255 miles, 737-300, Southwest Airlines

My closest flown to from PLH would be NQY, that's just 39 miles away, followed by BRS which is 90 miles away and my most flown sector at about 40/50 times now as it beats several hours on the train.
