Depends if you are flying international or not. If you are you have to pay, the rate varies according to how far you try to upgrade before you travel.
If you are flying within the US you don't have to pay, you just use your upgrades and 'join the line', your place in this line will depend upon your elite status.

In fact, if you're flying high yield economy fares (B and Y) the copay isn't required but for discount economy fares, yes, you need a copay.
From C to F the copay isn't required.
New York + Rio de Janeiro = One of the best combinations !
