Leaving out the European microstates with no airports (but easy access to nearby ones), I'd say that Nauru has this dubious honor. The island country's only air access - actually, access period, as there are no passenger ships - to the outside world is a single weekly roundtrip to BNE on Our Airline, the successor to the bankrupt Air Nauru. I can't think of any other sovereign country with such limited access.

What I find staggering is that they spent 30% of national GDP to buy the plane - or maybe it is per year to rent/operate it? But, when you are stuck on a rock in an Ocean, what else can you do.

Going to Nauru is even more difficult since they cut down from 2 to 1 weekly flight from BNE.

There are several other Pacific countries which have similiar conditions.

Kiribati is as far as I know linked to the very same flight from BNE to INU which continues to TRW. But there are 1 or 2 other weekly flights from NAN.

Also into the top 5 might be North Korea. There are very few regular flights, only from PEK, BKK, SHE and VVO. Not even a daily flight in total.

Until about 20 years ago Nauru had one of the world's highest per capita incomes due to massive phosphate deposits. Today most of the deposits have been exhausted, and the country's economy is severely depressed.
