Yes. IAH has 2 tram systems, one inside security and one outside, but to go from C to D I'm pretty sure you can just walk.

If you are arriving into IAH into one of the gates in the north half of C concourse, D concourse starts connected to C concourse on the north half. D1 thru D3 are connected to C concourse, D4 and above are still connected but further walk. If going out of D6 and higher, it would be easier to take the shuttle train inside security(connects to A, B, D, and E)

Nah, by the time you find the above ground train on the north side of Terminal C might as well keep walking down past D6, it is good exercise anyway!!
Quoting PHKLM (Thread starter): Is it possible to stay air-side when connecting in IAH from Terminal C (arrival) to Terminal D (departure)?
You may find yourself arriving Terminal E as well which also is connected to Terminal D and very easy to access the D6-D12 gates.

Quoting PHKLM (Reply 4): Thanks for the clear reply!
Not sure how clear, but it is very very simple once you look at the joint and walk it.
