I have. One time. In Atlanta. I was flying DCA-ATL-CMH years ago on EA. My flight from DCA-ATL was late (weather) and missed the ATL-CMH flight. I received a hotel voucher but I slept at the airport overnight -- and woke up to see my bag missing.

As I was just doing a little day trip, I just cut my losses and returned to my point of origin (YHZ, via YYZ).

Nearly. Our QF flight from ADL-SYD had been delayed, with only a 1 hour connection in SYD. Because of the number of passengers on board our flight, they delayed the SYD-BNE flight. You should have seen the evil looks some of the other passengers gave us.

I missed my flight from EWR to AMS (routed SBN-CLE-EWR-AMS) on CO because we sat on the tarmac at CLE for 3.5 hours!!! It was ridiculous, it was hot and they wouldn't give us any drinks besides water. However, there wasn't enough so everyone was only allowed about two sips. Additionally the FA were VERY crabby (to say the least). The "reason" for the delay was congestion at EWR, however, when we arrived it looked very empty (both people and aircraft). I was then rerouted via CDG, which was very fun as well. We barley made it home on time to get to the notary (as we were moving). In the end we were glad to be home (in time), and I was especially happy that i got to fly a Beechcraft 1900D.

About 2 years ago I was flying BLR-FRA-LAX-SMF with one hour to connect at FRA. The flight from BLR departed exactly one hour late due to the aircraft being late arriving (I believe due to snow back in FRA). While were still in the air Lufthansa had already rerouted those of us who were likely to miss our connections, and the crew announced our new routings over the PA. I was now booked on a United flight to IAD. Now actually getting a boarding pass for my new flight was the tricky part. That may have been partly my fault as I think I might not have gone where I was supposed to go and ended up going through immigration and ended up in the main lobby at FRA. But once I was there I found a Lufthansa desk and explained the situation. The lady behind the counter said she could see my new reservation, but couldn't issue a boarding pass and said to go to the United counter. They couldn't issue a boarding pass either, something to with the reservation existing but LH needing to issue the ticket first or something. She sent me to a different LH counter upstairs (I think maybe the actualy ticket sales desk) where at last someone was able to help me. She gave me a better routing (apparently my original new routing was FRA-IAD-LAX-SMF; she was able to route me FRA-IAD-SMF), got my checked bag rerouted on my new flights, and gave me a card to take to the United ticket counter. I went there, got my boarding passes, then through security (which was backed up), sprint to the gate, get there with minutes to spare, got my seat assignment, and got my my flight back home.

I was comming in from GOT to FRA and was put in the holding patern for 20 minutes so I just had 40 minutes to get from 1A to 1C to catch my flight to JFK. With queues on both the imigration and security controll I was at the gate when they announced the final call.

I lost count long ago. One great test of an airline's customer service is how they handle it. WN and UA sometimes have had it figured out in advance before I even get down. US has handled it so badly I won't fly on their airline again. LH will go to extraordinary efforts to help you make connections at MUC and FRA. Once I was about to miss a connection at FRA and they nearly pushed me into a car at the foot of the air stair and sped off to my connection. I insisted I was in no hurry, but they tossed me onto the outgoing flight, slammed the door and we left for Berlin. I got my baggage later that afternoon.
Inbound from Paris showing 45 minutes delay on airport screens. Still showing on-time for departure. Anyway, inbound arrives (320-100) and we leave from BCN about an hour late. Arrive in Paris. Too late, last flight of the day to OTP had left. No chance of making it. Lined-up for the rudest agent I've encountered. Got booked next morning via AMS (all 5 CDG-OTP the next day full, really? Gimme a break)... This was travelling with my grandma who had almost been run over 5 months previously so we needed a wheelchair. Bitch-agent told us to be at terminal 2F at 5am... It opened at 5:15, so I got too see CDG completely empty for once! Anyway, first flight to AMS... A320-100...delayed. Long delay. Unload the baggage of the Taiwanese circus troupe who hadn't showed up. Delayed still. The troupe gets there. Load the elephants and tigers back (joke there). Not very entertaining for a circus. Guess what, we then missed the flight from AMS to OTP too. KLM at AMS upgraded us to Biz-class on the very next flight on a 734.
Only time I missed 2 connections. Well, AF did. No, not on a journey that was supposed to take 24 hours, one that was supposed to be 5 hours long! Ended up basically being 24. Could've gotten to Australia in that amount of time.

Missed the connection in FRA and had to wait 5 hours because of a late departure at Heathrow and then slow security in FRA.
Massively jammed security in IAD C/D meant that it took 30minutes to wait and then 15 to clear security (the actual screening) UA was also 45minutes late from MUC. Missed all of my other options. Went and got a hotel room (no voucher) and a warm meal. UA offered me all sorts of crazy routings (eg on DL: DCA-NYC-DTW-OKC the next morning at 5am) but I just took the direct IAD-OKC the next afternoon.

Fortunately only a couple of near misses (at LAX and JFK) so far. When travelling on leisure purposes I tend to choose a longer connection, also to 'enjoy' the airport.
I have missed a self-made connection though. I was flying BIQ-LGW on U2, and then LHR-AMS on KL. I was supposed to have about 6 hours to connect trough London. The U2 flight ended up being about 5 hours late, so no way I could get to LHR in time. I called KL booking centre, and they rescheduled me on a later LHR-AMS flight. Fortunately my original booking class was also available on the new flight, so I only had to pay a change fee, and not a difference in ticket price. Bummer was that the original KL flight would have been my first flight on a Fokker 70, but I ended up in a 'boring' 737.
Off the top of my head, about 8 times in 2011. Every one had an airport connection window of at least 2 hours (weird, but I always seem to make the tight connections).
6 of the 2011 ones involved BW, one was CO, and one was 7I who were 'at fault'.
The CO one was weird (left early), 7I at least put us in a hotel in CUR (trust me, this is not a bad place to spend a day waiting for a flight...I'm going to fly with them more often just for the chance of similar delays).
BW is BW, so if you have no real choice on carriers, you just assume that flights will be late and connections are going to get missed on a regular basis. Sometimes they have help flights back to wait, meaning everyone is late (some of the glares I have gotten from other pax as we board those flights that were held back for us are purely evil!). And when delayed forcing an overnight, the late night ride into the city of POS usually takes forever for them to arrange, so if you are lucky you get to the hotel about two hours after arrival).
Anyway, if I got particularly stressed over flight scheds / late flights, etc., I'd just get different job - as it is, I like the job so deal with the grief (and the occasional CUR type layover isn't so bad either).

I did once: missed the 5:50 pm DTW-PIT last December because of strikes in CDG which delayed the departure of my CDG-DTW flight. I was rebooked though on the 9:30 pm one, and luckily I was because I got upgraded.

This happened to me a few times. Back in '97 we were flying MCO-ATL-LGA and we sat on our L1011 (!) in MCO for a few hours due to weather; DL put us in a hotel in ATL overnight. In 2008 flying JFK-IAH-MZT on CO, our 73G had a hydraulic problem and missed our connection; ended up going IAH-MEX-MZT with the final leg on MX!
Most recently, I was going JFK-DTW-ROC on DL Connection (Mesaba to Pinnacle) last December and my connection was supposed to be around 35 minutes. We were delayed out of JFK a bit, knocking it down to 12... at DTW a S5 Embraer "punked us" (as my seatmate said), pushing back from its gate and blocking us from entering ours. Connection down to 7 minutes. Finally pulled into the gate, I was at the very end of B and had to go to the very end of C... was literally 30 seconds too late

It happens from time to time, with the most recent one a few weeks ago. Was supposed to fly ARN-AMS-BOS-ATL-LAX but instead got changed to ARN-AMS-BOS-DTW-LAX as the flight from AMS was delayed by almost 2 hours.

The most recent time I remember (not saying there weren't others) was back in 2008 coming back SAV-ATL-MCI. It was actually supposed to be a 1-stop direct flight, but weather in ATL delayed the inbound plane to SAV. We left about an hour late, and in that time, DL found another aircraft and dispatched the ATL-MCI leg before we arrived. It didn't turn out so badly in the end though, since that leg bumped my mom into Silver Medallion territory, so we ended up in first class on our rescheduled flight.
I came close to missing another a couple years ago, since my OMA-ATL flight was delayed (somebody broke an overhead bin on our CRJ and it took a while to get someone to tape it shut). Fortunately I was still able to make my connection to SAV with about 10 minutes to spare.

Let's just say I have flown on CDG-OTP a lot and at least the TAROM flights were never full (we can call it insider info). I'm pretty sure she just didn't check the RO flights. TBH, it's not just that. If you're being nice about it I don't care... I know how Delta handles these things (proactively now), and I know how KLM handled this problem (flights full, no problem, let's try in C)... And it's completely different from how AF/CDG did. We weren't even trying to get a free flight or any compensation.
The only times ground agents pissed me off were this CDG thing and at O'Hare in 2000, which is another story, involving 5 journeys (well, 2 and a half) between terminal 1 and terminal 5, with only the SAS agent giving a crap (when our tickets were on OS and UA)... Eventually they sent us through LHR on UA then RO (that's the only time I was in LHR btw).
I have missed connections on DL numerous times, but never once did they treat me badly.

Last summer (June 18th to be precise). I was flying to SEA for an internship so I connected through CLT. Unfortunately (or fortunately, for a weather geek like me), there was a strong storm over the airport that just refused to move. During our first attempt at landing, I immediately recognized the sound of the engines powering up. The plane was shaking violently side to side, up and down, so needless to say that landing would have been suicidal. We circled the storm twice (the pilot was 'looking for an opening'). In the end, we diverted to RDU where we stayed for nearly an hour or so. The storm was headed our way and we took off just as it began to pound the airport. All US flights were delayed so I thought we might make it in time (10 of use were connecting to the same SEA flight). Alas, we landed 10 minutes late.
However, the service I received from US was VERY good. I was rebooked on first class (as an upgrade I paid in SJU) on the first SEA flight leaving CLT the next morning and received a voucher for a discount in a hotel. Unfortunately, I did not get much sleep that night since I managed to get some shut-eye at 12:00am and had to wake up at 4:00am for the shuttle back to the airport. Luckily, the first class seat and the 5 hour flight was all I needed to recover from the event.
Then, on August 20th, I ALMOST missed my connection, but this was my fault...and if I had arrived late, I still wouldn't have missed my connection. Let me explain: I flew SEA-PHL-SJU. After checking, I had an hour between flights and PHL has a Chick-Fil-A, a restaurant I love and one which I wouldn't get to eat in for a while, so I headed over to the food court and sat down. When I looked at the monitor, I had just 10 minutes before they closed the gate, so I swallowed the food in one bite and ran to the gate which was completely empty and issuing final boarding call. Made it in time: headed left for my first class seat on the 757 (I always wanted to do that: head in the opposite direction of main cabin passengers). There was just one thing: the captain from our flight was still in the air flying in from CLT. It would be nearly 30 minutes before the captain arrived and the door was locked, which was good since that gave me time to digest the late lunch in time for the excellent dinner that came during the flight. All in all, we arrived on time to SJU.

I almost did on my first flight of this year (Which was also my first, and probably last ever, flight on an MD11). I was flying PTY-AMS-PRG. AMS-PRG was a 1:00 connection which is more than enough at AMS ... that is when you land on time. Upon landing 20 mins late and on 18R Polderbaan ... I knew it was going to be tight.
To top it off in style, immigration agents were checking passports and visas upon disembarcation. After a 10 minute wait, around 20 mins left for my flight I was politely asked by the immigration agent to walk with him into a separate area for interrogation. After carefully inspecting my passport, he found it strange that a full-time university student had time and means to travel so frequently. The best thing was I told him I had about 10 mins to get to my connecting flight on Pier B (We were at gate E4) to what he replied "Don't worry, you'll have enough time". He was right in the end. I guess he must have known I can run pretty fast.
Other than that, I've missed a good few international connections in ATL and EWR thanks to the endless immigration lines and the whole nightmare of re-checking luggage.

Many times over the years, almost always due to either a) weather or b) a mechanical problem causing a flight delay. With coach travel really been a seat on the bus these days, how airlines respond is how I rate them for customer service. For airlines I have experience with missing connections: KL, excellent. AS, excellent. NW, good. DL, good. WN, good. US, so utterly appalling bad, that I won't get on another US flight. A few close calls in EWR, ATL and PHL due to massive immigration lines.
Best experience....15 years ago flying MFR-SEA-MSP. Late into SEA due to fog, 45 minutes after my connection to MSP should have gone, only to find it is still there stuck due to weather. Aircraft door is closed, air bridge removed. Agent calls the captain, who agrees to let me get on. Ever got on a full DC10, 45 minutes late after everyone else has been sat waiting? Boy, did I get some ugly stares, must have thought the delay was waiting for me!

I just missed another one tonight... Due to various IRROPS during the day, I planned a 32min connection in YVR (arriving from YWG, departing to YYJ). As luck would have it, we took a short maintenance delay in YWG, plus a slightly longer fly time and voila, missed connection!
Oh well, 1hr 30min until the next flight!

Numerous times. Finally I remembered what an exec at a company I used to work at said: "Even Rats Learn!", and I stopped trying to connect thru PHL. It's been a lot better since then.
But yeah, missed connections happen. You live with it.
(Actually, when I think about connections, the ones I made unexpectedly are more memorable than the ones I missed. The 55 minute connection CDG from 2D to 2A, with busy security; I was the last one on the plane. The 23 minute (!!) cross-airline, multi-terminal connection in ORD, and I have no idea what temporary idiocy caused me to purchase that one. The 75 minute, fog-bound, late-arrival-at-a-remote-stand, terminal 1-to-2 connection in FRA, with a busted leg on crutches, when I was frantically trying to hobble down a corridor to get to what seemed to be to be a 50+ minute security line, that was rescued by a maintenance worker opening a door and saying "Sie konnen durch hier gehen". Whoever you were, danke sehr....)

US DCA-PHX-LAX. WX at DCA delayed the departure over 1 hour causing a missconnect in PHX. The customer service desk nearset the arrival gate, already had everyone re-accommodated and boarding pass printed. I asked if I could change my destination from LAX to ONT and the agent was happy to make the change.

Twice. The first was in 2008. I was flying JFK-LAX-SYD-CNS on AA&QF. We had a 3-4 hour delay in JFK and missed our flight to OZ. I'll never forget it because I saw QF 108 take off in front of us out of JFK and thinking "Oh we will for sure make that one at least!" unfortunately it was booked solid. Ended up overnighting in LAX (which didn't end up being that bad) and caught the flight the next night!
The second was just last month. I was flying DL GFK-MSP-BDL to come home for the holidays from school with my buddy. We had a 519 flight from GFK, after a lengthy delay and an APU fire we lifted off something like 6 hours late. Needless to say I missed my connection (as a matter of fact my flight to BDL was on the ground before I left GFK, thats a depressing thought). My buddy and I were offerred hotel vouchers but by the time we got settled we would have had to head back to the airport. So we just grabbed a bench and rode it out. It was a rough morning but after you haven't seen home in over 4 months, it was more than worth it. (well the 7500 miles they gave me helped too )
