14,219 feet (4,334 metres), Airbus tested the A319 there, China Southwest flew B752 in there.
2nd highest must be LaPaz 13320ft/4060meters
Lhasa, Tibet is with 11,621 feet (3,542 metres) the 3rd highest commercial served.
Lowest is Furnace Creek / Death Valley National Park at -210 feet, -64 meters
, lowest international is Amsterdam at -11 feet (-3 meters).

How long the runway need to be for these fully loaded planes to get airborne from that airport?

Bangda Airport BPX
RWY Length is incredible 3.4miles/5500meters (should be a little more than 18000 ft, right?) making it most probably the airport with the longest rwy in the world.
CSWA - China Southwest Airlines should have regular flights into BPX from Chengdu with B757, Air China Southwest got especially equipped A319's capable to operate at runways at 4,419meters elevation - which is slightly above the BPX elevation for operations into Bangda. Most likely the majority of flights is seasonal for tourism. Bangda is also extensively used by Chinese Military flights.
I dont think that planes like 777/744 or A340 could operate there unless they are especially prepared. The A319 certified for this operation has high-thrust CFM56-5B engines (27,000 lb. Sea Level Static Thrust) in order to carry a full load into BPX. Testflights where also made with an A319 with CFM and IAE V2500 engines.
I will upload a few pictures later on today on my private webspace showing the airport with a 757 in this stunning environment - I have seen people asking for pictures from Bangda here earlier. Will be around 6pm GMT.

For info - Cusco, Peru is at around 3,300 metres.
La Paz, Bolivia - on the other hand - is actually built ABOVE the world's highest major city - on the high bit of town called El Alto...at a whopping 4,100 metres....I flew in from sea level in Chile due to bus strikes, and had trouble even standing in the terminal on arrival due to the elevation!! Incidentally, depending which direction you land from, La Paz is a pretty exciting airport. We came in in a LAN A320 from the city-side eg out over the chasm in the mountains that La Paz is built in, turn, and drop over the city to land square on the edge of the cliff above it that the runway is built on. Awesome!
