I hate to sound mundane, but it boils down to this for me: on-time performance and seat comfort. I don't want to miss my connections, I want to arrive on time, and I want to be relatively comfortable. When I think back on several hundred flights (almost all of them in coach), I say, "yes, that flight had a good seat and it arrived on time," or "I was squashed in that seat and I missed my connection." That's what makes it memorable for me.
If the flight attendants are nice, or if I receive a meal, or if the ride is smooth - this is just a bonus for me.

For me the other passengers are one of the main factors.
Easyjet for example is offering a decent product, but the crowd they're attracting on their UK-Germany routes is immensely getting on my nerves!
I'm trying to avoid Air Berlin for the same reason. It's not like LH is a lot better on german domestic routes, but they seem to attract a more sane kind of customer....
I've got nothing against screaming kids or most people for that matter, but sorry, I'm not interested in what you got, how awesome you are or how badly you'd "slam that b****" that just presented the aircrafts safety features to you! Morons...

But it all seriousness, all I want is the flight to meet my reasonable expectations. An airline doesn't have to exceed my expectations to make it a great flight. I'm perfectly happy if I'm well-received at the check-in counter, my flight boards/leaves relatively on time, the cabin crew are pleasant, my seatback pocket isn't missing the in-flight magazine, and my luggage doesn't get lost or rerouted.

For Business Class, It is important that I don't consciously feel like they are being cheap with amenities, food and drink options. Good Cabin crew should anticipate the needs of their customers rather than having to request everything. I enjoy a good wine list as well, Champagne or at least refreshment on boarding is a must..and I prefer a window seat and my QC15s.
If, like on my J Class flights CDG-CAI-NRT it was a cheap fare, then I tolerate more, so although the food was not so good on MS, the bed was comfy and and crew hepful and able to anticipate what I might like (offering to move me up to the non-family/infant cabin from my seat.... I gave MS top marks for those flights.
For Premium Economy: A window seat, on TK Comfort class product. Good amenities, good food, good wine, top IFE.
Economy. A window seat, a skinny seat mate who has no children/infants/mobility disabilities. I appreciate things like amenity kits, water bottles handed out, being given a full can and if they have IFE I like it to have enough to keep me occupied for the duration of flight. Generosity in general from crew I appreciate. A decent size meal tray (like EK/TK) goes a long way

I guess it will throw up the same quandary that airlines phase.. For my wife it's all about IFE, whereas I'll sit and look at the map all flight.
I like sideways seat space, so even european business class (which often has the middle seat blanked) always makes a great flight., however if you get a middle seat empty on any Easyjet flight I'll be equally happy.
The next one is a bit more contentious - if the missus reads this! - but it's sitting next to someone who you care about. There's always worrying that they're ok, and they're not about to hurl during turbulence, and I haven't got to explain 'what that noise is' - don't get me wrong, I love travelling with my wife, but my business trips are always more enjoyable flights!!
I on a more realistic note - in business it's all about exclusivity. On AF's 77Ws they have a small j class on the back of first which is fantastic, the main 50-60 seat section behind just feels like a comfy economy - and feel like it loses the sense of exclusivity - which lets face it is what the premium pays for. We're not really getting £2000 worth of extra food, wine and space are we? - it's all about that feeling. - maybe splitting up J if there's a big section would be nice?
Another thing, which I've only seen done once (again in J) was that we were taken from the lounge to the aircraft to board LAST. This was great as you're in the comfort of the lounge whilst econ is boarding, you walk on, and they close the door behind you (this was Malaysian from LHR where this happened). Never see why J wants to board first, only to sit there for an extra 30 mins!
I agree, the smaller the cabin the more 'exclusive' the feel, personally I dislike the large J cabin on BA 777 4 class ac as it has a bit of a dormitory feel to it, obviously passengers in First are going to have the best seats in the house, however Club World cabins could do with feeling a bit more private.
Premium passengers are allowed to board first for a few reasons, firstly to give them the most time to sort themselves out and make their way to the aircraft from the lounge rather than the feeling of being rushed at the end, feeling like a nuisance/holding up the flight.
Secondly it gives passengers a chance to settle into their seats, receive a boarding drink/newspaper, wash bag, slippers & PJs whilst giving the crew time to introduce themselves and build up a brief rapport with them as to what sort of service they want to receive (especially important in First).
Finally, it is a status symbol to sit at the front of the aircraft, likewise the earlier you board , the higher up the 'ladder' you are.

More or less in order, assuming economy class which is what I fly 99% of the time:
On-time departure and arrival.
No obstruction under the seat in front of me (specifically, no IFE box). I don't generally use the overhead bins.
A short security line with minimal drama and no little Hitlers yelling instructions or trying to show off their power.
A window seat with nobody next to me.
Either nobody in front of me, or at minimum the passenger in front reclines slowly and stops part way. No sudden slamming of the seat back.
Sufficient seat pitch; I'm not particularly tall but I hate having my knees rammed up against the magazine folder.
On longer flights with meals, something edible. (That excludes "pasta with white sauce" and similar horrors. I don't expect gourmet, but I do expect something better than boiled starch with a fake cheesy drip.)
Reasonably friendly gate agents and flight attendants. I don't care about FA service all that much but I at least expect something vaguely resembling an occasional smile.

Upfront, what I expect from the crew is a pleasant attitude and the willingness to do their job, as opposed to hiding in the galley after meal service as they do on some carriers. Removing trash and unnecessary utensils whenever possible is much appreciated, especially if I fall asleep with a half-empty glass of water positioned to wake me up rudely in case of excessive turbulence.
Walking the cabin regularly during long-haul flights, preferably with trays of water
Buzzwords like being proactive and anticipating needs are great. If a crew member shows up to take an order merely because they saw me go through the in-flight snack menu, that's fantastic, but it's not like I expect someone to monitor the cabin non-stop for just such a case either. Do answer the bell with reasonable timeliness when rung though.

It's not so much a case of being able to board first - premium passengers should be able to stroll on whenever they wish, but the idea that someone takes you from the exec lounge to the a/c is essential for a great airport experience. In Premium I expect to be able to hand myself over to you and not worry about a thing. For me, I'm always wondering in the lounge whether it's time to go or not, and that does detract from the experience.

Which is why for me a great flight is one where I have an entire row to myself. Then I can have the map on one screen while watching the IFE on another. And I get to sit in the window seat but can still go use the lav whenever I need to without having to climb over other people (while I like the window seat I try to get the aisle on longer flights just because I seem to have a small bladder so I like to be able to get up to go to the lav whenever I need to).

A clean aircraft cabin. I don't want the previous passengers garbage in the seat pocket or crumbs all over the floor.
Seat cushions that stay in place, Some will try to slide out from under you during the flight.
And NO disappearing flight attendants. I just hate it when they throw out a service right after takeoff and then spend the rest of the time in the galley discussing company issues. If you want a drink refill you have to find something to do with the tray that is still on your tray table and go to the galley to ask for what you want. UA in particular was bad about this. I believe that the F/A's think that if they leave your food tray on your table it will cut down on people moving about the cabin.

For me any hassle free flight, is a good one. But of course there are things that make a flight stand out:
- Since i'm quite tall, I always enjoy a flight with a little extra leg room. I have been quite lucky with LH to get an exit row on short haul flights, or have an entire row on long haul all by myself. For me this can make a huge difference on my overall flight experience.
- Getting a better service on-board than expected. For example, last year when flying FRA-AMS, which is a really short flight, I was served with complementary drink service, and a (really decent size) salad.
- The overall experience of the airport. Even though a flight can be really good, if your you have to wait a really long time at either security, immigration or baggage claim, this ruins the overall experience for me. Personally I hugely prefer the large airport, with a lot of things to see/do when waiting airside, above the smaller ones. For me, coming from The Netherlands, any flight starting from AMS (excluding the U2 flights that use their own, small, part of the airport) is getting a head start on my flights starting from EIN or MST.

switching things up here a little...What makes a good flight for me:
1) an affordable flight: brightens my mood when traveling knowing I got a good deal
2) short TSA lines: I have never been stuck in really long lines, luckily I haven't, I it is something I want to avoid dearly. TPA lines are pretty swift, and my worst experience was not that bad at ORD. Been catching some luck here
3) an empty seat next to me: While I like seeing full flights because that is usually good news, I would rather prefer my own comfort, and not have to deal with a distraction that can ruin my flight. This is just a preference, i have had good seatmates before, but if in doubt, I would take the empty seat. More personal space
4) Good comfort: similar to three, but just a nice aircraft, with good leg room, and not-so-tight seating.
As I travel economy on U.S. domestic flights, and basically travel only for vacationing, etc., my list varies from some of yours. I don't use lounges, and when I sit in economy, it is nice when the FAs smile and try to please my needs, but that would mean so much more if I was traveling business or first. For an economy traveler, just don't be grumpy as a FA, don't interrupt me through loud conversations with others, and just offer me extra stuff in eco and I will be a happy camper. Another bug on my flights is people who cannot sit still, and have to walk around, and sit at the back of the plane and talk, especially when I am near the back and I can hear their conversation. I have never flown on a long haul flight, so I believe that would be a different case, but domestic flights that are not all that long, it should not be too difficult.

I don't have any illusions about what flying commercial in economy actually is: public transportation. I'm not having fun, and that's ok...I paid for transport and that's all I want. So I really like it when cabin crews drop the whole formal act and just help me endure this. Joking around is great but not necessary, plain ol' natural conversation really goes a long way.
I can't stand the line "we invite you to sit back and enjoy your flight.." I can't "sit back." Damn thing only goes back an inch or so before I'm crushing the poor guy behind me. Enjoyment? After fighting traffic to the airport, waiting half an hour to get felt up by a federal rent-a-cop, then running to the gate only to find that there's a 90-minute ATC delay, there's no way I'm enjoying this. I'm quite satisfied if we're moving.
I guess a little acknowledgement that this whole thing can be stressful as hell goes a long way.
