I've only got three, but one -- Texas International (née Trans Texas) -- is not on anyone else's list (so far). The other two are Braniff(1) and Eastern. On second thought -- is Prinair still operating?

Quoting planeguy727 (Reply 5): When making this list I feel like the passenger that airlines should avoid...
Here's my list, and the first one, I didn't see mentioned. And I bet not many of you can list it, either!:
1. LEGEND AIRLINES, the one and only, best little airline this nation has ever known.
6. Transmeridian (the US based charter)
7. America West
and soon, #9, USAirways. For better or for worse.

I've also flown America West...but I don't consider it Defunct since in terms of corporate continuity, America West = US Airways. The name retired, the airline's executive, HQ, and culture remain intact
ATA, flatly died, the culture's gone, no HQ and executives.
I've never flown Pre-merger US, if I did I'd consider it "defunct" since HP took everything.
