I've got a wedding in Rome this summer and want to fly business or first from the US East Coast. I'd like your takes on who has the best premium service.
I'm familiar with the DL BizE (I take them several times a year to the UK and AMS) but that's about it.

I would reccomend flying on Swiss to ZRH in First, but make sure you get the A340 and not the A330 as the A340 offers a better product The flight will be from JFK
You can then pick up a connecting flight at ZRH to MXP flying on the RJ100
Swiss is an excellent airline, I reccomend flying them

There's also CO BusinessFirst, nonstop from EWR to either FCO or MXP. While I've never had the chance to fly it myself, I've repeatedly read that out of all the US carriers, it's far and away the best J service to Europe. You'd be either on a 777 or a 762, both of which would be relatively new aircraft.
