Thai does not have PTVs on their 777s, because they are regional aircraft. SQ, AF, RG, and AZ I'm pretty sure do have, and UA has on all aircraft not in the Hawaii configuration. CX I'm not too sure.
CX has PTVs on their A340s, and SQ used to have (they retired the A340s). Air France probably has, because the 747s (with the exception of the newest ones) are the only long haul aircraft in the fleet without PTVs.

Ive been on a Gulf Air A330 in economy and it had PTV's. My apologies to Olympic - i was told otherwise but the guy who told me was a turkish cypriot so that could be why!

The Austrian 340 are definitely not all equipped with PTV's. Last December/January January I flew on a 340-200 VIE-KIX-VIE, and this plane was definitely not equipped with PTV's (to make things worse, on the outbound flight, the audio system and reading light also refused to work, at least in my seat, and so the flight was veeeeeeery long.......).

Not all Austrian A340s have PTVs. Back in 1999 when I flew on OE-LAH A340-200, there were no PTVs in economy class, but I was lucky enough to be flying in Grand Class that time so it was no problem for me.
I think this is how AUA has fitted their A340s: (If anyone knows more than this, pleas correct me)

Thai first had PTVs in their 772s but later took them out (why ?).
China Southern has PTVs in their -200ERs.
PIA will have PTVs in their 777s from what I have read in pressreleases.
Khalifa never operated any revenue fligths with their 777s but BA, the previous operator of the aircraft had PTVs so it would have been surprising if Khalifa took them out.
United Airlines has PTVs in all three class 777s (54 out of 60 777s) and the two class mostly fly to HNL from US west coast or DEN.
Air Austral : does NOT have PTVs in economy class. They do in their Premium economy class.
Vietnam Airlines have PTVs in their first two 777s in economy since they are for longhaul. The next four will NOT have PTVs in Y.

LanChile has PTVs not only in thier entire A340-300 fleet, of the entire 767-300ER fleet too. Their PTV system is one of the best out there, all on-demand and with games.
