For the past 15 years or so, have Air New Zealand always routed via LAX on the Heathrow-Auckland run? If not, where have they routed via?

As far as I am aware (someone may prove me wrong), all the London-AKL flights with Air New Zealand have flown via LAX for at least 15 years, and I think even longer. These flights were also via LAX when they operated from LGW during the 80's before they moved back to LHR.

Yes, it used to operated AKL - PPT - DFW - LGW and vv

In 1988/1989 one of the three services went via Papeete and Dallas, and then in the final season Papeete was susbstituted with Honolulu.
Also somepoint in the mid to late 1990s, one of the three Frankfurt services was tagged with London, flying AKL-LAX-LHR-FRA-LAX-AKL, or vice versa.
