Today, it is a common practice for airlines to operate twin-engined aircraft across the Atlantic and even Pacific. The only airlines I can think of that don't do this are NW and VS (which will change when NW's A330's arrive). If I'm not mistaken, TWA was the first airline to operate a twinjet across the Atlantic, with a 767-200ER I believe.

Does anyone know the first route this was operated on?

I'm fairly certain it was El Al with a 767-200

According to the book Jetliners (©1993), the first trans-Atlantic flight operated by a twin-engined aircraft was a TWA flight, using a Boeing 767-200ER.

I would assume that this means that TWA made the first ETOPS flight.

As for what the flight was (ie, between which two cities), I'm not sure.

El Al made the first commercial non-stop Trans-Atlantic flight with a twin-engined airliner in April 1984. They operated a B767-200 between Tel Aviv and Montreal. However, the flight flew a non-ETOPS route.

After gaining small exemptions extending the non-ETOPS 60min rule to 75min, Air Canada, El Al and TWA all started using the B767 over the Atlantic during 1984. Using non-ETOPS rules you are forced to use a northerly route to keep within 60 or 75 min of diversion airports in Canada, Greenland and Iceland. Air New Zealand and Qantas were operating similar services in the Pacific.

The first B767-200ER was delivered in October 1984 to Air Canada, with additional redundancy and safety equipment in anticipation of the start of ETOPS services.

TWA operated the first revenue passenger ETOPS flight on 1 February 1985. TWA flight 810 operated from Boston to Paris on a converted B767-200ER, under the new ETOPS 120min rules. A route they were already serving with the B767, but ETOPS allowed a more direct route to be followed...
