It is a common practice to ferry a 5th engine on a 747. My guess they needed to replace an engine in SIN. There are numerous other posts on this topic. Try a search (If it has been fixed) or try a photo search.

The 747 can carry an extra engine under its wing root for the purpose of transporting the engine to engine shops for reaper or deliver an engine to an aircraft that needs an engine replaced.

The fifth engine actually does not provide power during a flight.
