What Charter Airlines Are Left In The USA?

I believe Champion still flies a bunch of 727s on a charter basis.

If you're counting freight operations, there's Kalitta, Evergreen, Arrow, Kitty Hawk and Gemini. Don't know if Zantop out of Willow Run (YIP) is still flying?

Hooters Air is really Pace. Apple usually uses Ryan, Transmeridian, and Champion Alex. BTW, I'm surprised you haven't chimed in on the STL-CUN thing  Big thumbs up

Primaris, Sky King and Blue Moon are also charter outfits I've crossed paths with.

Apple Vacations is a tour operator and not a charter airline, though they own there own airline that flies almost exclusively for them for the most part and that is USA3000. Also Vacation Express is also a Tour Operator and not a charter airline. Now most scheduled airline can and do charter work at times. Falcon Air Express and flights from RDU where done for a company called Grand Bahama Vacations, that right now is not running any charter flights to the Island of Grand Bahama as the Island was hard hit by weather related storms recently.

Lets nor forget SunTrips. They operate mostly out of Oakland, CA to Mexico, Hawaii, and Liberia, Costa Rica on behalf of several tour outfits including their own. They operate those funky DC-10-30s to Hawaii that you can see overnighting at remote stands at OAK. I know that they also ise Ryan International for some services though.
