Pay up to a K fare (if there are seats still available in the K inventory) and you'll be able to...many times, I've seen that K fares on domestic flights are only $10-20 more than an L, $30-40 more than a U. Of course, it depends on availability of upgrades, as all Medallions will be higher on an upgrade list than any non-elite. Soem routes are pretty hard to get a Medallion upgrade on, let alone a paid standby upgrade.

And never happens on transcons. Medallion upgrades are often very difficult to come by on transcons for FOs and GMs sometimes.

Whenever I've had to buy a K fare, I've called SMS (it's normally so I can have a companion upgrade).
As for upgrades, I'm a FO and am 1/2 this year on trans-con upgrades...I got the last upgrade on a Saturday afternoon ATL-LAS, while the return red-eye I was around number 10 on the upgrade list...only one Platinum got upgraded on that flight (F had 23 out of 24 passengers as revenue customers).
LAS is a tough airport to upgrade to...lots of conventions there, and a popular place to buy F fares (due to FL competing on the route, A fares are pretty cheap), so it's tough to upgrade into LAS.
