For most of the 1990s it was two returns each day, after it was brought back into service in Nov. 2001 the second daily was dropped.
BA001 left London at 10.30am arriving around 9am in New York
BA002 left JFK at 8.30am (I think) arriving in London at 5.30pm
BA003 left London in the early evening (used to come over my school near Reading around 7.30pm)
BA004 left JFK in the early afternoon arriving as the last scheduled flight into Terminal 4 at LHR at 10pm.
When BA announced they were pulling the Concorde flights in 2003 for the final summer they moved the BA001 departure to 6.30pm rendering it useless for those business passengers who were previously using it and rendering it much less useful for connecting flights..

There was often a spare Concorde at JFK. Whenever another flight 747 had a mechanical problem or major delay they often put the full fare first, business, and elites on that Concorde. It happened to me twice and a couple of times to a fellow I worked with in the early and mid-90s.
And, the counter people at JFK were really good at it. They would put on a sad face and apologize for my travel plans being disrupted. Then they would claim they had an alternative. They would then hand me a boarding pass (Concorde) with a slight smile on their face. Their apology would be about arriving in London several hours earlier than planned. One of these fellows said their entire job was worth seeing peoples' faces light up when they had that happen to them.
To me that was an ultimate in customer service recovery. And, it went a long way with loyalty that kept the revenue flow going to them.
Both services were year round however BA3/4 was not operational for certain periods of the year. For example in 2000 BA3/4 wasn't operational between 31Jul and 30Aug and in 2001 between 30Jul and 29Aug. During the W98/99 timetable the Saturday BA3/4 was only operational between 25Oct and 26Dec.

Ok there was a seasonal service to Barbados, but that was about it in the last few years of Concorde in service!
Well, BA only had 5 Concorde's flying after the AF crash, and each flight had to have another aircraft on Stand-by at LHR if the aircraft went tech, so 2 Concorde's would fly, 2 would be as back up, and the other aircraft would either be in maintenance, or on Saturday, going to Barbados.

I don't think any Concordes were flying commercially mid August 2000 until November 2001
