Garuda Indonesia is leasing 13 aircraft this year, including 4 B747s, 4 B767s and 5 A330s, in addition to which one Garuda frame will be dispatched to the Hajj operation. The airline will also reinforce its mailine operations into JED. Garuda will be carrying approximately 104,000 pilgrims in 275 flight groups to both Jeddah and Madinah starting next Tuesday, November 28. The last return flight will be operated on February 01. It's an operational nightmare I am not particularly looking forward to.

PIA has leased atleast 2 Tristars, 1 Malaysian B777-200 1 Thai Orient B743 to my knowledge for Hajj.PK hasnt bothered with a hybrid colour scheme or anything as all plane retain their original colours.

Your list is correct, except for Surabaya, which is entirely served by SV with up to 8 daily B747s and sees no Garuda flights. Garuda will deploy an A333 for the Hajj from both Banda Aceh and Medan. Pilgrims from Manado and Mataram (Lombok Island) will be shuttled through domestic flights to Makassar (UPG) and Surabaya (SUB) respectively. Batam (BTH) serves as a stopover point.

PIA have leased MH 777 for their own use not Hajj, they have also leased one A310 and one A321(In full PIA scheme with flag tail).
