This is straight from American Airlines Corp. HQD:

AMERICAN’S FLIGHT 1291 FROM MIAMI TO PORT AU PRINCE RETURNS TO MIAMI AFTER RECEIVING A FIRE WARNING LIGHT

FOR RELEASE: Monday, Nov. 20, 2000

FORT WORTH, Texas – American Airlines Flight 1291, an Airbus A300 operating from Miami International Airport to Port Au Prince, Haiti, with 121 passengers and a crew of nine, returned to Miami shortly after takeoff this morning after a fire warning light came on in the cockpit.

After making a normal landing, the aircraft stopped at the end of Runway 9L and passengers and crewmembers were taken off the airplane using evacuation slides. Five persons, including one flight attendant, were reported injured during the evacuation process. The flight attendant was flown to Jackson Memorial Hospital and has died. The flight attendant’s name will not be released until family members are notified.

The cockpit warning light came on as the airplane was climbing through 14,000 feet. The airplane took off at 11:49 a.m. (eastern time) and was back on the ground at 12:17 p.m. The passengers will be flown to Port Au Prince on another flight later this afternoon.

Nothing has been reported about a F/A being sucked out and killed.

What kind of injuries can you sustain from evacuating an a/c that would kill you? Unless the F/A did in fact get sucked out, which I didn't think was possible on the ground.

Associated Press - - -

POLICE SAID THAT as the crew prepared for the landing, one of the crew members opened an emergency door before the cabin was depressurized.
The crew member was pulled out of the plane and died on impact on the tarmac.
Police say six to 12 other people suffered injuries during the landing.
Authorities have said the plane was taking off for Haiti when an engine fire light came on. They said the plane landed, and that passengers were evacuated on emergency chutes.
An airport spokesman says there were 130 people on board.
Authorities had said earlier that the plane was taking off for Haiti when an engine fire light came on and the plane returned to the airport. Passengers were evacuated on emergency chutes, authorities said.
