This is something I didn't know about until I saw this story, apparently in the report on the United Airlines 811 fatal accident where the 747's cargo door opened itself in flight, the NTSB had placed blame on one of the ground crew workers for failing to close the door properly, despite that it was an electrical fault that caused the door to open. Just recently, the employee mentioned in the report has finally been cleared of wrongdoing.
