I, too, have thought of that question.
In my opinion, air service to that part of the island would be a great idea. RVR has the potential to grab the entire eastern catchment area. The residents on the eastern side of the island do not have easy access to (major) air service and the drive to either SJU or PSE is a pain.
AA would be good flying it from MIA, maybe even JFK. But as we know, AA has not only virtually abandonded the PR market, but considering that AA is mostly interested in business/premium centers & traffic, I dare to say AA will never fly this route --- at least not in my life time. Pluss AA in Ch.11 now, my bet is that they will cut more, instead of add, cities in the immediate future.
However, the best contender would be B6: from JFK, MCO, and even FLL. They have the traffic & could further develop it even more with connecting feed from those cities. Additionally, if B6 really wanted to, they could set up an interline agreement with Vieques Air Link, Cape Air, or whomever to feed traffic to from VQS, CPX, etc.
NK also could do it to FLL and/or MCO, but I see B6's potential much larger & stronger.
So, I believe the potential is there. Getting someone to actually do it, especially in this current economy, is another story.
In the meantime, I'll keep my fingers crossed.

I am 100% with you on this one. The resorts like El Conquistador, Palmas del Mar and the Westin will welcome any carrier that lands at RVR... Fares from SJU are to high, too much traffic and Redeyes from PSE and not healthy for the tourism..

I feel like this airport could be like LRM in the Dominican Republic. While La Romana and Bayahibe are close to the major airport of Punta Cana, roads make for slow driving to the area's resorts. With that being said, LRM is grossly undeserved from the US. AA from MIA (an ER4) and B6 from JFK are the only scheduled carriers from the mainland. Although the airport has TONS of charters from Canada, UK, Germany, Benelux, Spain, and Italy.
Puerto Rico does not have the number of resorts of the Dominican Republic, therefore struggles to bring the large number of tourists.
