Yes its the design which makes it better IMHO . The catering is your standard LX lounge snacks. Not bad but nothing amazing.
The LX lounge at BSL was unfortunately timed. It was designed to serve the old Crossair hub at BSL which was inherited by Swiss and then shut down in conjunction with LX disposing of all their ex-Crossair Embraer 145s and Saab 2000s. The lounge is now far too big for LX's current operations at BSL and is usually almost empty. The last time I used it it was also starting to look like it needed renovation, especially the wood floors.

This reminds me exactly why "rankings" of this sort (like SkyTrax) are usually utter rubbish. They are entirely subjective, and only look at a few lounges as possibilities - I doubt the author of this "ranking" has been to all the lounges of the world to be able to compare them.
Personally, my favorite lounge by far is the TK Business Class lounge in IST. The TG lounge in BKK is also pretty nice.
However, I have never been in a F class lounge, and I haven't even visited 1/10th of the thousands of lounges in the world. So therefore, I won't authoritatively call the TK lounge "best in the world" - I'll merely say that it's a quality lounge.

I too have visited the Star/TK lounge in IST and found it be absolutely one of the best I've visited. The catering was amazing in terms of quantity, quality and diversity. Moreover, the dark theatre room was a nice place to grab a quick nap. And, despite the fact that they only have the one lounge, it has never seemed too crowded.

I am surprised that the JFK Clubhouse is #1, yet the flagship LHR Clubhouse (though getting a bit tired) is absent from the list.

Been there last Saturday morning and it was packed to the roof. Could not find a seat. It's a great lounge in general but can't be compared to the First Class lounges on the list.
I wasnt expecting much when I was there back in December but was really pleasantly surprised. Liked it alot.

Indeed I think when its packed it takes away from the enjoyment , if you cant find a seat you get a bad view of it.

Regrettably, I am not first class passenger, so I most likely will not experience most of those lounges. However, one I am looking forward to is the new AF lounge in S4 at CDG. While I know it will not stack up to the likes of LH First or QR Premium terminal, I do think it will be a nice change to some of the rather dreary lounges found at CDG.
A work colleague has already passed through it and loved it. Has anyone here given it a try?

The AF 1st class lounge at CDG is missing also. As said by other posters, this kind of ranking is very subjective and might be questionable. The only thing for sure is that these listed lounges are all nice.

I haven't been to all of the mentioned lounges, but I have been to quite a few of the top ones. I have to say that SK's lounge in CPH is quite up there and the co-branded SK/AC lounge in LHR is nice as well.
