Yes. Anywhere. There's quite a bit of airspace in eastern Russia that aren't avaliable to equipment having only 60 mins ETOPS

Yes they do...many airlines use twin jets over the Atlantic, SIA, United, and Qantas use to on they SYD-YVR route.
