Business on United, and Air New Zealand. those are just the only airlines I have flown on the upper deck before. I know there are alot of different set ups out there.

I think it differs on the airline concerned

Mostly J, although BR has both F and J on the Upper Deck on B744s. Qantas has Y on the B742s

It depends on the airline but mostly Business some like Lufthansa First Class.

Depends on the airline. Some of the Virgin Atlantic aircraft have Premium Economy on the upper deck, and some have Upper Class up there.
