The MD-11 Combi - the variant with the side cargo door in the rear part of the fuselage - is to my knowledge only used by Alitalia. Why is this? Is the type uneconomical or simply not designed good enough to take advantage of both passengers and cargo (read: too few passenger seats and not enough cargo space).

Keesje: Martinair does not use the variant that Alitalia has in service. Martinair and World Airways uses the CF-version which has the side cargo door in the forward part of the fuselage. Just like the old DC-10-30CF´s used by SABENA in both cargo and passenger configuration. Alitalia´s MD-11 have the SCD to the rear of the fuselage and the cargo pallets behind the passenger cabin as on the B747 Combi. You can see the difference in these pics:
