I live very close to PIT, and we're lucky to get two or three A330s a day.

When I lived in PHL, we got a daily A340 when LH came in from FRA.

At SFO, I've seen SQ, LH and AF A340-300's on regular flights. I believe that SQ may drop the A343 on the ICN-SFO route by this fall (replaced by a 777-200ER) and AF might fly the A343 on the CDG-SFO route again after the summer season ends.

HKG must be the 'city of the A340'! We get:

Cathay Pacific (loads everyday for obvious reasons)
China Airlines
Virgin Atlantic
Air Mauritius
Sri Lankan
Air Canada
Gulf Air
Lufthansa
Philippines

And I believe we will be getting the following in the near future:

South African (-600)
Virgin (upgrade to -600)
Air Canada (upgrade to -600)
Cathay (extra -600s)
Swiss (when their M11s get replaced by 343s)

See my homepage for a comprehensive guide to spotting and photography at HKG

At ORD, I see several A340's from various airliners.

At MNL, most A340's seen at any one time undoubtedly belong to Philippine Airlines.
