It happened while building the new university children hospital in Basel, Switzerland.

To the left is the centre for pharmacology and molecular biology (Pharmazentrum/Biozentrum) of the University of Basel. I studied there in until June 2007.
Edit: The accident happened because the crane driver disenganged the overload C/B. At the current configuration, the crane could safely lift 12 tons, but the load was 14 tons.
