Even though this has been dragging on for quite some time now, I think a final deal is on the horizon for upto 20 A330 tankers in a deal worth £13 billion.
The European AirTanker consortium, led by aerospace group European Aeronautic Defence & Space Co, has been named preferred bidder for a 13 bln stg deal to build refuelling aircraft for the UK's Royal Air Force.

Apart from superior tanker capasity it will give the RAF a long range strategic transport capability they haven't had before, meeting new globalization requirements.

Spacepope,
There are no plans to add a freight door up top. The current cargo holds are considered sufficient, though they will be modified to take the NATO pallet.
The main deck will be modified to take 40 stretchers if required.
