Yes, did it for 3 years. First year the pay is crap. Second year it's bearable. Third year, call STA travel and pay your own travel - then the pay is ok. There's Bunacamp as well - I tried both and they're both pretty much the same.
Go to the camp fair in London - I think it's around Jan-Feb time. Pick private camps as they have the best facilities. Don't worry if it's a Jewish camp (there are loads) - I didn't find it a probem being non-Jewish (same applies for other religions I guess).

No, you have the qualification before you even apply for the job (read: before the fair). Essential if you want pool duty; not essential otherwise, but can be desirable at sports camps. Oh yeah, there are sports camps, and arts'n'crafts camps - they're the two main categories.
I see you're under 21 (legal drinking age in the USA). Pick a camp in a US State that allows 18-20 year olds into bars, as you've got a chance of getting an alcoholic drink! Pennsylvania doesn't, New York state does. Dunno about the other ones.
