All three are factors, but methinks primarily the first one.
Yes, unions can and do get overzealous, customers increasingly want more for less, but while you hear of service being cut and employees laid off when an airlines funds take a turn for the worse, you never hear of executive bonuses being cut - crappy and selfish management is the biggest problem (and I know that this problem isn't restricted to the airline industry - it's everywhere).
