If an airline has one airframe/engine certified for etops how difficult is it to get certification for different engine on different airframe model eg US had Pratt A333 and now RR A332. Do they have to start from scratch in terms of building up hours on the new airframe/engine combo? Is it only the engine which is relevant here?

The maintenance program is already in place and the airframe requirements are already established. Time matters with the specific engine but it certainly is not from scratch. Nowadays many of the requirements are about the airframe and not just engines.
If you have never designed an airplane part before, let the real designers do the work!
