The aft section of the fuselage, referred to as the 'tailcone' has an access door and a platform that opens up right under the engine with a couple of bench style seats. This allows access to many of the engine's components. The tailcone itself is hinged at the bottom, and the entire tailcone can be pivoted downward to get it out of the way for lowering the engine when it is being removed. For maintenance on the rear or top of the engine, a lift is necessary. Hope this helps.

At Delta, we have a lift truck that we park underneath and behind the tailcone which raises up to engine level and then slides underneath it. This truck is used for nightly layover checks, for simpler checks, we use the "Patio" that Boomer described.
