If an instrument approach is possible, why is it not mandatory? Seems that this happened only because the pilot got confused by what he saw. Why rely only on your eyesight, if you can also be aided by your instruments.

The plastic cylinder is in between the two outer pressure panes, as you can see in the photo. It is not attached to the inner pane that is inside the cabin. Why would air get in between those two pressure panes?

It doesn't surprise me at this time of year. If a 747 say has an airspeed of 565 mph, that would mean, with that tailwind going eastbound, it would have a ground speed of 800 mph. Which, would be supersonic in relation to ground. LOL. Not bad.
