Score! Kid Born With Three Arms

Wed May 31, 2006 2:29 am

Extra digits are always a blast, but an extra limb? That's awesome!

SHANGHAI, China -- Doctors in Shanghai on Tuesday were considering surgery options for a 2-month-old boy born with an unusually well-formed third arm.

If the extra arm is functional he could have an advantage.

Seriously though, does the kid live downriver of a polluting plant or something? He looks pretty healthy other than the semi-shock of seeing an arm where there isn't supposed to be one.

If the arm stays he could make a good goalie (soccer or hockey), baseball player, football receiver, etc...as the list goes on and on.  Wink

I think we need Alex's expertise here.

He does look very healthy.

I wonder how the movement of the arm is, and if taking it "off", will it hinder, in some way the use of his two other arms? Maybe the brain, overtime, will adjust and in the future will not recognize that it was there when born.

I'm blabbering and will stop now.

That's pretty much a guarantee, as your brain isn't fully developed until you're ten years old and this kid's probably only months at most.

Theres a girl down the road from me (roughly 8) who has only 2 big toes and 2 little toes and her left hand is sort of normal after a few ops and her right hand is well you cant tell how many fingers she's got.

There are many plusses and negatives to having 3 arms but i think the best thing is to cut it off.

I wouldn't say that. It is either 1. a sign that there was another embryo, that regressed at some point and left a "part" attached to the sibling. Similar to what happens in Siamese twins. Sometimes there is even a rare condition called fetus-in-fetus, in which a small, deformed, hypoplastic baby is found inside the abdomen of a newborn.
or
2. This is a sign of severe environmental pollution, which brings about mutations and malformations like these. Cyclops, anencephaly, and so on. Particularly in China such malformations are becoming increasingly common.

Quoting 9VSPO (Reply 3):
Will they remove one?

Certainly. It is necessary to bring the child back to normality, particularly so that the above attached limb can function properly. The problem is how to remove the extra-numerary limb without compromising innervation (i.e. brachial plexus) and vascularization of the "good" limb. Such social stigma is a rather bad thing for a child. And by looking in the arm, it might be rather difficult since it seems to possess an intact joint at the ribs.
Interesting case, actually. I would love to operate on him...  Smile

That is one interesting case... if only the article had mentioned whether or not he has trouble controlling one or both of his left arms.

Basically (very much so) one part of the cortex on either side of the brain controls the limbs on the opposite side. So I'd be very much surprised if he had full independent control of the left arms, my guess would be one arm dominating and the other one being either highly dysfunctional, somewhat synchronous with the other or able to execute muscle contractions the other one isn't capable of.

The complexity of the neural "wiring" leaves lots of space for awkward (dys-)functions; if I were a neurosurgeon, this boy would be somewhere between my greatest challenge and my worst nightmare.

Oh don't be so dramatic. A pylocitic astrocytoma in the brainstem would be 1000x more difficult. Actually, you don't need to be a neurosurgeon for this OP. Plastic surgery would be more helpful.

If you come to hospitate with us you can assist me in some supra-numerary digit amputations, to get a feeling for it, OK?

Wouldn't there be a third scapula (shoulder blade) for that arm? In my studies in human anatomy and embryology, I learned a lot about the embryonic development of the limbs. I would think that as the limb bud grows out from the embryonic body, the bones would form in order from most proximal to most distal (first, the scapula, then the humerus, then the radius and ulna, and finally the hand bones). How else would the third arm attach to the trunk?

Also, from my studies in embryology, wouldn't that third arm have formed due to an ectopic limb-bud signal being expressed in a spot where limbs do not normally function? I remember reading about some past experiments on amphibian embryos where limb bud tissue was grafted into an ectopic spot, prompting growth of an extra limb. Would environmental toxins (eg. teratogens) have caused a mutation in a homeobox (pattern-setting) set of genes, leading to mis-expression of limb-patterning genes in the "wrong" area?

What you said makes a lot of sense, alas I'm no longer able to discuss it so I can just agree but not add to it. The two scapulae might be conjoined.

I will still be at the ICU, unfortunately. I will be back as a full fledged surgeon only from October onwards. After that, anytime. However, if you really want to come in july, I can IM you the address of my boss, to whom you have to address your request (of course I will put in a good word for you...)

Quoting SmithAir747 (Reply 30):

Over an ectopic joint, which does not necessarily involve a scapula. By looking at the child, it seems that the extra-numerary humerus is attached to the glenoid cavity of the normal scapula.

Quoting SmithAir747 (Reply 30):
mutation in a homeobox (pattern-setting) set of genes, leading to mis-expression of limb-patterning genes in the "wrong" area?

Whoa! hold on your wild horses! That is a great theory but not quite yet confirmed. Homeobox genes seem to interact with both inhibitory (MASH vs NOTCH) and excitatory (CREB) pathways, so you could deduce that their activation could produce mixed results. Maybe going a step down the cascades (JAS pathway, for example) or even looking at VEGF - FGF - IGF expression may provide the right answer. Don't forget that growing limbs on Axolotl and other species is much simpler than in humans.
But a nice take on the problem - you will be a great scientist.
