lists hotels from 5 star down to budget and rates are cheap (less than chain rates). You'll also find that people have rated them and also read their comments. Have used this site many times and had no problems. You'll be wanting a hotel in the Bukit Bintang area.
Most of the high end chains are represented.

2) If there is a car with a driver to stay with us during our stay there.
A good idea, I wouldn't drive in Malaysia...they're insane.
Your hotel concierge can also arrange a car and driver. The hotel I stayed at a few times organised a taxi at an hourly rate, which was good enough for me - to get me around the 'hard to reach on foot' sights in KL
itself and then a trip out to Batu Caves the next day. The rest of the time I walked around the areas or rode the monorail. Got to see quite a bit of KL
Shopping, shopping, shopping...
Bukit Bintang area has a multitude of malls and also over at KLCC Suria at the twin towers.
As far as everything else, that depends on your interests;
The National Museum is good for an hour or two.
For a great overview of KL
try the Menara Tower (but get a taxi to that, it's a killer walk up the hill in the heat).
Chinatown - both day and night
The bird park is also nice to walk through and there is a garden across the road.
A daytrip down to Melaka, well worth it.
To get to and from the airport use the KLIA Ekspres
and on arrival avail yourself of their VIP Service
(pre book), with transfer by limo from KL
Sentral to your hotel.
