Climbing Back Onto The B747SP?

Hey everyone,

I just came across this picture...

My questions are, why did the flight engineer not in the aircraft for pushback? Why did the B747SP has a small exit just behind for the nosegear for? Where wil the flight engineer end up after gonig up that hole? I find it quite amazing about this... any other aircraft has this?
