Pilot Took Off From The Taxiway, Not The Runway!

From the news I've read so far, the pilot
was cleared for take-off. However, he
used the taxiway to take-off... probably
thinking it was the runway... However,
they did say the pilot flies on the
JFK-ANC-TPE route regulary on the A340s
and no one knows how he could have
made the mistake at ANC...

The US NTSB has already started investigation.
