U.S. Airport Operating Costs

Which U.S. airports have the highest and/or lowest operating costs in terms of how much it costs for an airline to operate out of the airport?

Charlotte has the lowest hub costs per passenger at around $1.25.

Pittsburgh, Denver and Orlando are among the highest, but I don't know exact costs.

What about SFO, LAX, JFK, and ORD? Are they among the highest?
