Flaking Airplane Windows

What causes the interior airplane windows to get all flaky? It makes looking out of it very difficult not to mention photography. But some dont flake! I was on a 737-200 and the window was very clear, The flaking seems to be mostly on the inside (cabin) not in between the panes.
