EgyptAir flies to New York (and in the past operated to Los Angeles) with 777 / A340 equipment.

MH is Malaysia Airlines, Malaysia Airlines is from Malaysia and Malaysia it self, well, I don't think that it is in the Middle East.

But yes, MAS still offers the KUL-DXB-EWR vv. run.

All airlines you listed have already been mentioned and as I said before, Malaysia isn't a Middle East Airline. Same for Cospn, I didn't know that having Arabic subtitles does make a US carrier a Middle Eastern one ...

Sevenair
I doubt that there would be enough market for US carriers to fly into the Middle East. Middle Eastern carriers have might have more political interest than economical interest in flying to the US.

I think 9/11 and that part of the world's proclivity to bombing and hijacking planes in the 70s, 80s, and 90s, put a damper on US Airlines' interest in serving the Middle East. I really don't think lack of passengers is the problem.

MAH4546,

Royal Jordanian doesn't operate any A340-300s. They operate A340-200s.
