How To Get UA Upgrade LAX-JFK Or EWR

I usually fly LAX to EWR to get to NYC. I'm a Premier member and I normally get seated in the Econ. Plus section when I buy coach. However, when I recently flew a LAX-JFK red-eye (coinsidentally on 9-11), I got upgraded to business class without doing anything. Business class was very empty on the flight.

Why did I get upgrades on LAX-JFK and not on LAX-EWR? Is it just because of the flight was empty and I'm Premier? Did it have to do with 9-11?

But, why wouldn't I get one on a LAX-EWR when there are business class seats empty on that flight.... does anyone know the answer?

And what is the best way to get an UA upgrade without using your miles? Is it possible?

I am sure it was because J was empty. Econ Plus is a given if you are premier, no matter what ticket you buy, but if you are premier and they have an empty front cabin, they will stick you up there.
