Don't forget the most important of all:
001 F-WOWW Airbus prototype, first to fly
and
021 will also be for Qantas

Yes MEA-707 , i really don't forget F-WWOW , but it is not allocated yet to an airline.
Not sure 021 will go to QF , that's why i didn't mention it.

Why have 4 slots gone to eithad when the aircraft wont' be delivered till 07? surely it makes more sense to put SQ, EK or QF orders on their first.

Wasn't the first frame built just for static testing while the second frame would actually be the first one that flew?

There are two static frames built which are 'hors serie', with different 5000 and 5001 frame numbers, 001 and up will be flying aircraft.

The reason Etihad could get 4 'early' slots is the first few aircraft will remain at Airbus relatively longer for tests. Also because they have been used for tests I guess Etihad gets a nice price for them, while Singapore and Virgin for instance preferred "fresh" frames which weren't banged around too much.
This is common practice with other Airbus and Boeing types as well.
For instance A-340s fuselage 2, 3 and 4 remained at Airbus a few years and were then sold to Virgin and the Saudi Government, while fuselage 8 was the first delivery to Lufthansa back in 1993.

Emirates 's selection of the Engine Alliance GP 7200 will delay its deliveries as the RR engine will be the first to fly and be certified( airframe/engine combination).
Etihad did not officially specify its engine choice but choosing the RR variant might get it an earlier delivery than Emirates
