There seem to have been a high number of inflight failure and susequent shattering of cockpit windows in Canadair Regional Jets.

The abnormally high rate of cockpit windsheild/window failures on the CRJ are the result of a flaw in the fabrication process. PPG (the manufacturer) has identified the problem and has changed their procedures accordingly. I don't know of any windows actually blowing out entirely while in flight, which is amazing considering how the whole pane usually spiderwebs. Very scary.
