Avianca 707 (lsf PA, 1960-61)?s

According to the book Boeing 707 / 720 by George W. Cearley, Jr., Avianca leased a 707-121, N711PA, from Pan Am from October 1960 to October 1961 to operate their Bogota-Miami and Bogota-New York (via Jamaica) services while awaiting delivery of their own 720Bs.

Was the aircraft operated on a wet-lease basis with Pan Am pilots and Avianca cabin crew, or was it flown by Avianca flight deck crew?

Since my production list shows that Avianca returned N711PA to PA in February 1961, did another PA 707(s) take its place from then until October 1961?

During the lease, to what extent did the 707(s) leased from Pan Am appear in Avianca's livery?
