Airblue Flight 202 Cfit Report

Thought there was an "Air Crash Investigation" episode about hierarchy in the cockpit. Sad that this cockpit was suffering from a dominant, irrational captain who doomed the aircraft and all 152 on board.

What can an airline do to monitor the performance of it's flight crew? There ought to be signs of this behavior on a daily basis that can be identified and the personnel evaluated.
